Breast problems are one of the most underestimated and overlooked of all female problems, particularly in Indian society. However, some fibroadenomas grow big and when larger than 5 cm. in size, they are called giant fibroadenoma. Such big fibroadenomas require removal by plastic surgery. Apart from fibroadenomas, there are other noncancerous tumors of the breast like phylloides tumors, lipomas (tumor of fat cells) sebaceous cyst (tumor of skin), galactocele (milk duct cyst), abscess (pus collection) and post injury tumors. Breast surgeries are done under general anesthesia. To maximize the safety of anesthesia, detailed pre operative work up is carried out. Fibroadenoma is a benign (non cancerous) tumor of the breast which mainly affects both breasts of young girls. However, this can happen to middle aged ladies also. Fibroadenoma of smaller size, up to 2 – 3 cm. usually do not require treatment, even though they are multiple in numbers. However, when one or more fibroadenomas enlarge in size, they require removal by surgery. Before removal, the diagnosis of fibroadenoma can be made sure by FNAC or biopsy, whenever needed.
Breast augmentation and breast reduction are aesthetic surgeries. They are not advised to the patients on medical grounds. Whenever an adult female patient presents to a plastic surgeon with complaint of less developed breasts or even if the breasts are normally developed and she wants to increase her breast size, a plastic surgeon can perform her breast augmentation surgery after proper counseling, thorough examination and necessary investigations are done. Phylloides tumor is a non cancerous tumor of breast. Though this tumor is not cancerous, it is firm to hard in consistency, painless and grows with speed; features resembling malignancy (cancer). To clear the doubt of malignancy before the surgery, tru cut biopsy (cutting a core of tissue from the tumor and submitting it for examination under microscope) is performed from this tumor. After confirmation with biopsy and other investigations, phylloides tumor is removed with a layer of surrounding normal breast tissue. Because this tumor has the propensity for recurrence, close follow up after surgery is advised.
Gynecomastia (male breast) is treated at Cutis hospital by an operation called liposuction with gland excision. Gynecomastia has two components; fat and breast gland. Fat is removed by liposuction and after fat removal, breast gland is removed via the same incision by procedure called direct gland excision. This operation is done under general anesthesia and patient is discharged to home on the same day evening. Patient can join his office after 48 – 72 hours of surgery. However, patient is advised to strictly avoid gym, heavy exercises, weight lifting, running, swimming and other out door strenuous activities for 4 – 6 weeks after surgery. Breast asymmetry is a congenital problem which is revealed only after the breast development is completed. Asymmetry of breasts is not an uncommon problem. Asymmetry of breasts is a cause of great concern and anxiety especially when one breast is very small and the other is quite big, bulky and ptotic (hanging down). Such patients are examined in details and the different operative plans are made for both breasts so as to match their size and shape as much as possible. In such circumstances, small breast is augmented with silicone implant and the large, ptotic breast is reduced in size as well as lifted.
