Breast Problems or Disorders Overview

Congenital breast problems have been present since birth, but many of them are not revealed until the actual breast development at puberty. At Cutis Hospital, breast problems of males and females are dealt with by an expert and qualified plastic surgeon in an efficient way. Cutis Hospital is the best hospital for breast problems.

Common Breast Problems

Breast problems are one of the most underestimated and overlooked of all female problems, particularly in Indian society. The reason is, that females usually avoid talking about and asking for help and another important reason is the lack of awareness among consultants themselves. Plastic surgery is a wonderful specialty that can address all noncancerous problems of the breast including reconstruction of the breast after its removal for cancer. Common noncancerous breast problems include fibroadenoma, fobroadenosis, phylloides tumor, breast abscess, and multiple aesthetic problems of the breast like underdeveloped breasts, breast asymmetry, ptosis of the breast, virginal (benign) hypertrophy of the breast. Congenital problems, though accounting for a smaller number of cases; include tuberous breast, nipple inversion, nipple hypertrophy, accessory breast, accessory nipple-areola complex, axillary breast and Poland’s syndrome. Other acquired problems of the breast include asymmetry, developmental problems, or deformities of the breast due to trauma, burns, or injuries sustained to one or both breast discs in childhood.

Types of noncancerous breast tumors:

Most common of all noncancerous tumors of the breast is fibroadenoma of the breast. It is a freely mobile, painless tumor that usually affects one or both breasts of young girls and most of the time they are plenty in numbers. These tumors always do not require surgery. However, some fibroadenomas grow big and when larger than 5 cm. in size, they are called giant fibroadenoma. Such big fibroadenomas require removal by plastic surgery. Apart from fibroadenomas, there are other noncancerous tumors of the breast like phylloides tumors, lipomas (tumor of fat cells) sebaceous cyst (tumor of skin), galactocele (milk duct cyst), abscess (pus collection) and post injury tumors. Plastic surgeon has the required training and expertise to deal with all such noncancerous tumors of breast. In fact, any approach by a plastic surgeon is invariably associated with the preservation of the aesthetics of the breast.

Types of congenital problems of breast:

All congenital (present since birth) problems of breast are not visible at birth. Many of these problems reveal themselves only at the time of breast development (puberty). Various congenital breast problems include accessory nipple areola complex, accessory breast, axillary breast, inverted nipple, tuberous breast, breast asymmetry, hypoplastic breast and breast hypertrophy. Poland syndrome is a congenital problem leading to non development of chest wall muscle and breast also. All these congenital breast problems are treated at Cutis hospital by our experienced and qualified plastic surgeon.

Types of Aesthetic breast problems:

Patients consult a plastic surgeon mainly for three types of aesthetic breast problems. These include breast hypoplasia (underdeveloped breasts), breast hypertrophy (excessive growth of breasts) and breast ptosis (saggy or fallen breasts). In plastic surgery, there is no repetitive formula for correction of deformities/problems. Correction of all aesthetic problems of breast is based upon fundamental principles of plastic surgery. And accordingly the operations are breast augmentation for correction of breast hypoplasia, breast reduction for correction of breast hypertrophy and breast lift for correction of ptosis of bteasts.

Breast Treatment Procedures

As stated earlier, there is no single procedure or select number of procedures for all set of breast problems. Plastic surgery procedures are based upon the basic surgical principles and the same surgical procedure can be changed and modified for two similar looking problems. For example, the correction of breast hypoplasia (underdeveloped breasts) requires corrective surgery. Now, this can be achieved by either insertion of a breast implant behind the breast (breast implant) or by injection of patient's own fat in her breast (autologus fat transplant). Even the breast implant procedure differs in insertion of the type of implant (silicone, saline, polyurethane), pocket of insertion (above the muscle or below the muscle) and size of the implant. Patient parameters and plastic surgeon’s clinical decision guide the treatment of choice for breast problems.

Risks and Safety of Breast Surgery:

Breast surgeries are done under general anesthesia. To maximize the safety of anesthesia, detailed pre operative work up is carried out. Investigations like mammography, sonography of breast and FNAC help the surgeon in planning and ensuring safety of the surgery. Planning is an integral part of success in plastic surgery, and breast surgery is no exception to this rule. In fact, success of breast surgeries like breast reduction and breast lift depends a lot on pre operative planning. However, all surgical procedures have some complications. In breast surgery, minor complications like minor wound gap, discharge of the clear/fat cells from the wound, delayed healing can happen sometimes. Red and somewhat raised scar (hypertrophic scar) in early stages of healing can also become a cause of concern. Major complications like wound infection and skin necrosis are fortunately uncommon. Post operative pain is comparatively mild in almost all breast surgeries except breast implant which can cause moderate pain and discomfort for initial 2 – 3 days.

    FAQ's

Fibroadenoma is a benign (non cancerous) tumor of the breast which mainly affects both breasts of young girls. However, this can happen to middle aged ladies also. Fibroadenoma of smaller size, up to 2 – 3 cm. usually do not require treatment, even though they are multiple in numbers. However, when one or more fibroadenomas enlarge in size, they require removal by surgery. Before removal, the diagnosis of fibroadenoma can be made sure by FNAC or biopsy, whenever needed.

Breast augmentation and breast reduction are aesthetic surgeries. They are not advised to the patients on medical grounds. Whenever an adult female patient presents to a plastic surgeon with complaint of less developed breasts or even if the breasts are normally developed and she wants to increase her breast size, a plastic surgeon can perform her breast augmentation surgery after proper counseling, thorough examination and necessary investigations are done. Same is true for breast reduction surgery.

Phylloides tumor is a non cancerous tumor of breast. Though this tumor is not cancerous, it is firm to hard in consistency, painless and grows with speed; features resembling malignancy (cancer). To clear the doubt of malignancy before the surgery, tru cut biopsy (cutting a core of tissue from the tumor and submitting it for examination under microscope) is performed from this tumor. After confirmation with biopsy and other investigations, phylloides tumor is removed with a layer of surrounding normal breast tissue. Because this tumor has the propensity for recurrence, close follow up after surgery is advised.

Gynecomastia (male breast) is treated at Cutis hospital by an operation called liposuction with gland excision. Gynecomastia has two components; fat and breast gland. Fat is removed by liposuction and after fat removal, breast gland is removed via the same incision by procedure called direct gland excision. This operation is done under general anesthesia and patient is discharged to home on the same day evening. Patient can join his office after 48 – 72 hours of surgery. However, patient is advised to strictly avoid gym, heavy exercises, weight lifting, running, swimming and other out door strenuous activities for 4 – 6 weeks after surgery. A customized pressure garment is to be worn by the patient for 6 weeks after the surgery.

Breast asymmetry is a congenital problem which is revealed only after the breast development is completed. Asymmetry of breasts is not an uncommon problem. Asymmetry of breasts is a cause of great concern and anxiety especially when one breast is very small and the other is quite big, bulky and ptotic (hanging down). Such patients are examined in details and the different operative plans are made for both breasts so as to match their size and shape as much as possible. In such circumstances, small breast is augmented with silicone implant and the large, ptotic breast is reduced in size as well as lifted. The planning of these two different surgeries on each breast is done with great details by our experienced plastic surgeon at Cutis hospital, Still, patient should understand that grossly asymmetric breasts can seldom be matched as naturally symmetrical breasts.
